每系1份,依各系需求辦理,若有訂定要點需送校級實習委員會備查

系級學生職場實習委員會設置要點
Guidelines for Establishing Departmental Student Workplace Internship Committee
每系1份,各系應成立本委員會,設置要點需送校級實習委員會備查(系主任擔任召集人,其成為應包含學生與業界代表至少各1)
One copy per department. Each department should establish its committee. The specific guidelines should be submitted to the university-level internship committee for record. (The Director of the department serves as the convener. The committee should include at least one student and one industry representative. )

2.實習機構以政府登記核准立案、具良好職場管理制度之公民營機構為原則
The internship institutions should be government-registered and approved, and generally private institutions with sound workplace management systems.
3.第一次合作之實習機構派員實地評估並填寫(實習機構可能因每次合作情形而有不同評估結果,建議於每次合作前重新評估)
For the first-time collaboration with an internship institution, it is necessary to send representatives to conduct an on-site assessment and fill out this form. (The evaluation results may vary based on each collaboration, so it is recommended to reassess before each collaboration.)

The contract should include the following details: internship period, total hours/weeks of the internship, student's department, job tasks or projects, internship salary or scholarship, accommodations and meals, pre-job safety and health training, working hours, rest periods, leave policies, insurance matters, internship mentoring, and evaluation system.
3.境外實習則應簽訂中、外文版「職場實習合約書
For overseas internships, a "Workplace Internship Contract" should be signed in both the local language(Chinese) and a foreign language version

2.學生參與公民營機構委託本校之專案計畫,其計畫執行期程需至少半年以上,且每計畫以四位學生參與為限,實務專題課程不得認列實習畢業門檻
For students participating in project plans commissioned by private institutions, the project's execution period should be at least half a year or longer. Each project is limited to the participation of four students. Practical project courses cannot be recognized as fulfilling the internship graduation requirement.
實習前說明會
Pre-Internship Orientation
得包含實習規定、實習評量、工作要求、性別平等、安全衛生、勞動權益、職場倫理或實習經驗分享等,以及職場實習合約書相關細節(如:合約載明之商業機密保密條款、賠償責任等)
The document should include internship regulations, internship evaluation, job requirements, gender equality, occupational health and safety, labor rights, workplace ethics, and internship experience sharing. Additionally, it should cover relevant details of the Workplace Internship Agreement. (Such as clauses regarding confidentiality of commercial secrets, liability for damages, etc.)
辦理學生保險
Arrange Student Insurance
1.系應辦理保險,保險額度不得低於教育部當年度實習學生團險規定(新臺幣200萬元意外保險及5萬元醫療險)
Each department should arrange insurance for the students, and the insurance coverage should not be less than the Ministry of Education's current regulations for student group insurance (NTD 2 million in accident insurance and NTD 50,000 in medical insurance).
2.教育部校外實習學生團險請參實習就業達人網置頂公告。
For the Ministry of Education's off-campus internship student group insurance, please refer to the announcement on GOOD JOB website of NKUST.
航輪二系海上實習學生需自行投保(新臺幣300萬元意外險及醫療險20萬元),若各公司有團保從其保額規定
The students of the Shipping Technology Department and Marine Engineering Department who are participating in maritime internships need to obtain their own insurance (NTD 3 million in accident insurance and NTD 200,000 in medical insurance). If the companies have group insurance, the students can follow the coverage regulations provided by the companies.
合法入境簽證、役男出境程序
Valid Entry Visa, Exit Procedure for Military Service Personnel
1.學生須妥符合實習目的之合法入境簽證
The students must obtain a valid entry visa that aligns with the purpose of the internship.
2.役男出境程序
Exit Procedure for Military Service Personnel:
(1)4月內短期出國,學生自行至移民署網頁申請
For short-term trips abroad within 4 months, students need to apply on their own through the immigration bureau website.
(2)4個月以上申請出國,由學校以公文書方式提出相關證明文件向戶籍地直轄、縣市政府提出申請
For trips abroad exceeding 4 months, the school will submit relevant supporting documents to the local city or county government through official documents to apply for the permit.

學生若發生性別平等爭議、薪資/工時爭議、實習適應不良、意外事故、境外實習緊急事故,應第一時間通報系(所)及輔導老師,由學校輔導老師查證,並與實習機構共同商議改善方案後輔導學生,並填妥本表,相關規範如學生職場實習管理實施要點第4
If students encounter issues related to gender equality disputes, salary/hour disputes, poor internship adaptation, accidents, or emergencies during overseas internships, they should immediately report the matter to their department (or school) and the guidance teacher. The school's guidance teacher will verify the situation, discuss improvement plans with the internship institution, and provide guidance to the student accordingly. The relevant details should be recorded in this form, following the guidelines specified in "Point 4: Student Workplace Internship Management Implementation Guidelines.
職場實習轉換及終止紀錄表
Workplace Internship Transfer and Termination Record Form
1.學生提出轉換實習機構需求,處理程序依學生職場實習管理實施要點第2點辦理
If a student requests to change the internship institution, the processing procedure will follow the guidelines specified in "Point 2: Student Workplace Internship Management Implementation Guidelines."
2.學生因無法完成實習而發生終止實習事件,系(所)應與實習機構進行解約並了解終止事由,並填妥本表,作為後續辦理實習機制之參考
If a student is unable to complete the internship and a termination of internship occurs, the department (or school) should terminate the contract with the internship institution and understand the reasons for termination. The relevant details should be recorded in this form, serving as a reference for future internship procedures.
