If the counseling mechanism proves ineffective, you may proceed with the termination procedures and should fill out the "Student Workplace Internship Termination and Transition Record Form“ when necessary.
If it is considered to terminate the contract with the original internship organization and transfer to a new off-campus internship organization, you will need to complete the necessary internship procedures again (please refer to the "Internship Planning Schedule and Related Forms" section). After confirmation and distribution by the department-level Student Workplace Internship Committee, you can proceed to re-sign the internship contract and individual internship plan.
